FKPM Rules and Regulations

Terms and Conditions:

In consideration of being allowed to participate in the activities and programs provided by the Fargo Indoor Play Museum Waiver Form (hereinafter referred to as "FKPM"), I, the undersigned, hereby acknowledge and agree to the following terms and conditions:

Assumption of Risks: I understand that engaging in activities within FKPM involves certain risks and dangers, including but not limited to slips, falls, collisions, and other potential injuries inherent to such activities. I am voluntarily participating in these activities, and I assume all risks associated with my child's participation.

Release and Waiver: I hereby release, discharge, and hold harmless FKPM, its owners, employees, volunteers, agents, and representatives from any and all claims, demands, actions, or causes of action arising out of or related to any loss, injury, damage, or death that may occur while my child is participating in activities at FKPM.

Medical Treatment Authorization: In the event of an injury or medical emergency, I authorize FKPM and its staff to seek and obtain medical treatment for my child as deemed necessary. I understand that FKPM will make reasonable efforts to contact me before seeking medical treatment, but if such contact cannot be made, I give my consent for necessary medical treatment.

Photography and Publicity Release: I grant FKPM the right to use photographs, video footage, or likenesses of my child for promotional and marketing purposes without compensation.

Supervision: I understand that FKPM is not responsible for supervising my child during their visit. I agree to supervise my child and ensure their appropriate behavior and adherence to FKPM's rules and guidelines.

Responsibility for Personal Belongings: I acknowledge that FKPM is not responsible for any lost, stolen, or damaged personal belongings.

Applicable Law: This waiver shall be governed by and interpreted in accordance with the laws of North Dakota. Any legal actions arising from this waiver shall be brought exclusively in the courts of North Dakota.

I have read and understood the contents of this waiver form and agree to its terms and conditions on behalf of my child. I acknowledge that this waiver is binding and shall be effective from the date of signing.
